eRate Letters of Agency (LOAs) This is the form that allows the State to request service on behalf of schools.  The State will need this form before they can post a state RFP for telecommunications or Internet services as a part of Phase II of LB 1208 implementation.  This form needs to be submitted by August 31, 2007.
