Once you file your Form 486 it is advised that you check with your Service Providers to confirm that they will be applying credits on invoices for the current funding year.

Following is E-rate contact information on select vendors that provide service for ESU 10 schools AND need to be contacted in order to confirm that credits will be applied to invoices. This is NOT a complete list of Service Provider Contact information. Please be sure to include the FRN# and any account numbers that your school wants the service provider to credit.

If your service provider is not listed below you can get contact information for your Service Provider's Erate department at: http://www.sl.universalservice.org/Forms/SPIN_Contact_Search.asp.

Service Provider's names change frequently so it might be helpful to have the Service Provider Identification Number (SPIN from your original Form 471 application) handy to use when you search for the Service Providers contact information.

When sending requests to your Service Provider it is best to send the following information in your request:
    School Name
    471 Application Number
    Funding Request Number
    Form 486 Number
    Service Provider Information Number
    Invoice Account Number
