FORM 470 FILING
If you have not yet filed your FY2009 Form 470, you need to do so now. Remember that your Form 470 and your RFP, if you have one, MUST be up for at least 28 days. We suggest that you file your form online and use your PIN to submit and certify the form.
Program Deadlines in October: On or before October 29, the following forms must be submitted online or postmarked:
* Invoices (Form 472 and Form 474) for FY2007 recurring services.
* Forms 486 for FY2008 services that started on July 1, 2007.
You should refer to the Deadlines Tool and the appropriate letters from USAC to calculate or verify your deadlines for these and other program forms.
Tech Plan Revisions -No Later than October 1st.
Please be sure that your tech plan revisions include some mention of the services that you are receiving E-rate funding for. Free and Reduced Lunch Report-October 15th
Please BE SURE to send ESU 10 a SIGNED copy of this report as we need it to document figures to the SLD during the application reviews. Instructions for generating an Erate Report on the NSSRS system can be found at: http://www.nde.state.ne.us/TECHCEN/documents/ERATEVALIDATIONREPORT.pdf
Form 472-BEAR October 29th
BEARs for most services delivered during FY2007 (July 1, 2007 to June 30, 2008) MUST be filed. Applicants should remember that paper submissions require service provider authorization prior to submitting to the SLD for payment. Early submission of reimbursement forms helps avoid delays during the service provider review process and allows extra time to resubmit a form if necessary.
